RHINOPLASTY
Rhinoplasty is the most demanding facial plastic operation. The variables and intricacies of human anatomy and genetic influences on all levels of healing make planning, preparation, surgery, and post operative care in rhinoplasty a universe unto itself. Minute change or major change, reduction or augmentation, primary or secondary, no rhinoplasty operation is preordained as easy or simple. Each rhinoplasty is a total emersion of every nuance of knowledge and experience to assure the best possible result. The surgeon must understand not just the external appearance, but also the internal nasal anatomic relationships. Once the examination is complete, a plan for aesthetic correction is presented to the patient. Dr. Pastorek likes to be absolutely certain of the patient's goal and have the patient know what is realistically possible with aesthetic nasal change before there is any discussion about preparation or the postop course. There are differences in the way various surgeons manage rhinoplasty. He performs his procedures through internal incisions. Patients usually leave the postoperative recovery room within one hour following the surgery. Cold compresses to the eyes are routine. The external silicone splint is removed in the office on the seventh day. Because of surgical edema the nasal appearance at the time of the splint removal is never the final result, but even at seven days the patient can expect the nose to look better than the original. Most patients feel comfortable returning to work or school immediately after the splint is removed. Dr. Pastorek feels that this rapid recovery is one of the benefits of the internal approach to rhinoplasty and attention to detail before the surgery. The use of NASA technology specific light wave frequency treatment after the splint in removed quickens the reduction of postoperative swelling. Usually 25% of the post op surgical swelling is down in 7 days, half is down at 6-8 weeks, and almost all swelling is gone by 6-8 months. After the visit to remove the splint the patient is usually seen at 60 days for a post op exam, and at 6 months for a final visit. Other appointments are made as necessary. In-depth information about postoperative care and activities is provided at the time the splint in removed. Post op photos are usually taken at 6 months.

BLEPHAROPLASTY
Blepharoplasty or aesthetic surgery of the eyelids improves the aging and baggy appearance of the eyelids. While the looseness of eyelid skin and eyelid muscle can be an effect of age, heredity, and sun damage, bagginess of the eyelids can be a pure expression of heredity in a young patient. The appearance of sleeplessness, tiredness and exhaustion often brings the patient to the facial plastic surgeon for a rejuvenation procedure. In the case of the young patient who has inherited a relatives baggy eyes, the patient just wants to look normal. Dark circles, fine skin wrinkles, and tiny eyelid skin lesions may be a part of the baggy eyelid problem. These require special attention and treatment. Any medical cause for the eyelid appearance must be evaluated.
Like rhinoplasty, blepharoplasty is aesthetic surgery of extreme accuracy and delicacy. The surgeon must be highly focused because of the required symmetry required of blepharoplasty. The anatomy and nature of the eyelids make secondary surgery problematic. All effort must be made to get the best possible result with the initial surgery. Years ago Dr. Pastorek developed a suspension suture technique to permanently support the lower eyelid. It allows for tightening of the lid, moves a small amount of upper cheek laxity upward, and prevents displacement of the eyelid assuring that the position and appearance of the lower lid is normal.
The ease of local anesthesia required for totally pain free surgery of the eyelids and the small surface area involved make the office OR environment ideal for blepharoplasty. Most patients leave the office within 30 minutes following the procedure. The patient is instructed to rest for the remainder of the day, use cold compresses, and enjoy a quiet day of music or books on tape. Discomfort is minimal. Sutures are removed on the 4th post op day. Make-up can be applied directly to the incisions on the 6th post-op day. Most patients return to business on the 7th day. Activities and cautions are discussed at the time of consultation and at the time of suture removal. The final result of blepharoplasty is realized at about one month post op.

FACELIFT/NECK LIFT/BROW LIFT
Facial rejuvenation surgery includes facelift, neck lift, brow lift, and blepharoplasty (as described above). Each patient seen for facial rejuvenation is evaluated for his or her particular need. Some patients will require several procedures to reach their optimal appearance, some patients will require one. No matter what plan is necessary, the goals are always the same; the most normal appearance, the most long lasting result, the safest surgery, the finest scars, and a rapid recovery and return to life's routine. Patients who can benefit from a brow elevation or a procedure to gain symmetry of the brows are evaluated as to the best approach for their individual need. Dr. Pastorek believes in a conservative and realistic brow appearance. Effectiveness and longevity of the browlift are paramount in the planning of the browlift. As a teacher and lecturer Dr. Pastorek studies and considers the latest developments in this field. He feels that the parade of new and inventive facial plastic surgery procedures offering short cut, minimal effort, and quick fix pathways to rejuvenation must be evaluated over an extended time period to test their worthiness and long-term effect.
Facial rejuvenation surgery is usually performed in the outpatient operating room under intravenous analgesia. A moderate pressure bandage is removed in the office on the day following surgery. The operative area is inspected, the wounds dressed, light wave treatment is administered, a "preview" of the result is presented, and smaller pressure bandage applied. The bandages and sutures on the face and lids are removed on the fourth post op day. Any sutures at the hairline on the seventh day. Staples are removed on the tenth or eleventh day. Make up can be at any time following bandage removal.
